Understanding functional constipation

Functional constipation is a disorder of brain-gut interaction in which colonic motility, rectal sensation and the coordination of evacuation become dysregulated — without any structural blockage or metabolic cause. Many patients also have dyssynergic defecation: the pelvic floor muscles paradoxically tighten instead of relaxing during a bowel movement, making evacuation mechanically inefficient no matter how soft the stool.

Why laxatives alone often disappoint

Laxatives address stool consistency and transit — useful, but they don't restore rectal sensation, correct dyssynergic patterns, or calm the gut–brain signalling that suppresses normal motility under stress. That's why so many patients escalate doses over years and still strain.

How brain-gut treatment helps

Psychological therapies and behavioural retraining address the upstream drivers: normalizing the gut's stress response, restoring attention to (rather than suppression of) the urge to defecate, and unwinding the toilet-related tension that perpetuates dyssynergia.
