Gut-Directed Hypnotherapy vs CBT: Which Is Right for Your Gut Symptoms?

Both gut-directed hypnotherapy and CBT are among the most evidence-supported treatments for IBS and other disorders of brain-gut interaction. They work differently — and for many people, the best answer isn't one or the other.

Two proven paths to the same goal

Both therapies target the brain-gut axis — the two-way signalling between your brain and digestive system that becomes dysregulated in IBS and DGBI. Both aim to reduce visceral hypersensitivity and calm the nervous-system arousal that amplifies gut symptoms, and both are recommended in major gastroenterology guidelines. The difference is in how they get there.

What gut-directed hypnotherapy does

Gut-directed hypnotherapy uses focused relaxation and gut-specific suggestion to directly down-regulate how your brain processes signals from the gut. You don't need to analyze your thoughts — the work happens at the level of automatic, physical responses. In clinical trials it improves symptoms in over 70% of IBS patients, with benefits lasting years. It often suits people whose symptoms are intensely physical, or who find talk-heavy approaches draining. See the evidence behind it and how it works.

What CBT does

Cognitive behavioural therapy targets the thoughts and behaviours that unintentionally amplify gut symptoms — catastrophizing about pain, food avoidance, constant symptom-checking, and the anxiety-symptom-anxiety loop. It builds practical coping skills and gradually reduces the fear and avoidance that keep the cycle going. CBT tends to suit people whose symptoms are closely tied to anxiety, stress, or avoidance patterns.

How to choose — or combine

It rarely has to be either/or. Many patients do best with a blend, and acceptance and commitment therapy (ACT) adds another option. The right mix depends on your symptom profile, what has and hasn't worked before, and your own preferences. That's exactly what an assessment is for — matching the approach to you rather than guessing.

What we recommend at our centre

We start with a Rome IV-based assessment, then tailor a plan drawing on hypnotherapy, CBT and ACT — adjusting as we track your symptoms over time. You don't have to decide the approach up front; that's our job.

Which works better for IBS?
Both have strong randomized-trial evidence and similar overall response rates. The better choice depends on the individual, not a universal winner.
Is gut-directed hypnotherapy legitimate?
Yes — it is one of the best-studied psychological treatments in gastroenterology, with response rates above 70% and durable benefit, and is recommended in major IBS guidelines.
Are sessions virtual?
Yes. All assessments and therapy are delivered online to patients across Manitoba.
Do I need a referral?
No. We accept self-referrals and respond within two business days.
